WHAT IS INCONTINENCE?

STRESS INCONTINENCE?
A common treatment for stress incontinence is Kegel exercises which gradually strengthen the pelvic floor muscles by regularly tensing and relaxing them. These can be effective, but are time-consuming and difficult, with slow progress, and for women with particularly weak pelvic floor muscles, are often not possible.
Traditionally, Kegel exercises have been recommended as the most effective non-invasive means of improving pelvic floor muscle strength. Unfortunately, it is difficult for many to identify the correct muscles to work on and maintain the necessary focus. Consequently, the success rate with these exercises can vary anywhere between 30-90%. For those with more severe forms of incontinence, Kegels are not helpful.

During the Treatment
During your session, all you have to do is take a seat comfortably on the Emsella chair. It is in a private section of the clinic so you can have privacy as you go through the treatment. There isn’t any need to undress or change into a gown, so you can stay in your clothing and remain seated for a 28-minute session. You may experience some tingling but there is no pain or other discomforts. You can bring a book or something to listen to that will keep you occupied for that duration. We do ask that you wear loose clothing such as loose light trousers or skirt so there are no restrictions on your body by clothing whilst the electromagnetic technology performs contractions on you, so please try not to wear jeans or tight trousers. What Are The Effects of the Menopause?
The effects of menopause on the genitourinary system (the system that includes the bladder and urethra) can be far reaching and have a big impact on the lives of menopausal and post-menopausal women.
During the menopause, there is a drop in estrogen, which affects many areas of the body including your skin and hair, the metabolic system (which causes that pesky weight gain), and the genitourinary system. It also slows down the rate of collagen production in the entire body, including the vaginal canal, which is one of the most common contributors to incontinence in menopausal and post-menopausal women. This leads to a thinning of vaginal tissue and slower tissue regeneration, causing even more issues for women, such as vaginal dryness and increased looseness in the area, known as vaginal laxity.
When a variety of symptoms in the genital and urinary systems begin to appear – including symptoms like pain during sex, vaginal dryness, an increased susceptibility to UTI’s and stress incontinence – it is known as Genitourinary Symptoms of Menopause, or GSM. GSM affects around half of menopausal and postmenopausal women, and, unfortunately, only gets worse over time, meaning it won’t resolve without treatment.

The Effects of Childbirth on the Body.
The effects that pregnancy and childbirth can have on the body are varied and can have a huge impact on your life, both while you’re pregnant and after you’ve given birth. These changes range from weight gain and changes to your body shape, to psychological differences and changes in the thyroid, skin and hair. For example, different areas of the brain actually grow and shrink during pregnancy and childbirth, and many women find they have skin breakouts after giving birth because of the dramatic changes in hormone levels! Pregnancy and childbirth affect everyone differently, but the vast majority of women will experience incontinence or urinary leakage to some degree during pregnancy or after childbirth.
During pregnancy, the weight of the baby pressing on the bladder can actually disrupt the way your bladder and pelvic floor muscles (which are the muscles responsible for holding and controlling the uterus and urethra) usually work, leading to symptoms of stress incontinence. Vaginal birth can also affect the pelvic floor muscles, due to the strain it puts on the pelvic floor. Vaginal birth can weaken these muscles, making difficulty holding your bladder or some level of leakage much more likely. You’re not alone though, as an estimated half of women experience changes in continence after childbirth.
Pregnancy and childbirth can also cause damage to the nerves controlling the bladder or displacement of the bladder and urethra, and having an episiotomy during labour can also contribute to incontinence and leakage among postnatal women.
What is Postnatal Stress Incontinence?
Postnatal stress incontinence is very common, and is caused by the stress put on the pelvic floor during childbirth. While normally, the pelvic floor muscles wrap tightly around the urethra , and only relax when we consciously decide to, after childbirth, they (understandably!) become stretched and looser. This means that when stress or pressure is put on the bladder, for example, when you sneeze, cough, laugh or lift heavy objects, there isn’t a barrier to prevent any little accidents.
For most women with normal births, there is a certain degree of leakage that is expected, but normal continence should be regained after a few months. Factors increasing the likelihood of women developing severe postnatal stress incontinence, include having several babies vaginally, a long or problematic birth, obesity, smoking, the birth of heavier babies and giving birth over the age of 35.
Pelvic floor exercises such as Kegels are often recommended to women suffering from stress incontinence or difficulty regaining control of their bladder after having a baby, but they can be time consuming and the process of strengthening the pelvic floor with these exercises is long. We also find that many of our patients cannot feel their pelvic floor muscles in order to contract them so cannot carry out kegel exercises at all.

POST-NATAL INCONTINENCE
Postnatal stress incontinence is an incredibly common condition, even among women with normal births and no particular complications. This isn’t really surprising when you consider the strain that vaginal childbirth puts on the pelvic floor muscles. When the pelvic floor muscles are working normally, they wrap tightly around the bladder and urethra to help hold the bladder tightly closed, and only relax when you consciously make the effort to.
